What Is Included?

• Treatment for the upper 6 front teeth only.
• Treatment can be single crowns, bridges, veneers and/or a combination.
• First appointment with a Treatment Coordinator for a preliminary consultation and x-rays as needed (also included in the fee).
• A consultation with a doctor for diagnosis.
• Three to five total appointments required.

Additional Important Information:

• This package price does not include the treatment of decay, abscesses or any other tooth problems.  
• If additional procedures are needed, they will be quoted at our normal fees.  
• These same guidelines apply to any additional procedures.  
• There is no warranty for this treatment. 
• A 20% non-refundable deposit is needed to schedule the treatment appointment. The remaining balance is due in full the day treatment is started. 
• Forms of payment accepted: cash or check, NO EXCEPTIONS.
